The Partnership Journey
Rachael Fox, Registered Manager at All About Home Care explained that Mrs H had been a client of AAHC for over 12 months. Her needs and conditions were becoming greater than what daily visits could provide and she had always expressed a wish to stay at home. So AAHC recommended that the family contact their trusted partners, The Good Care Group, who could provide live-in care.
Rachael comments,

As part of the partnership agreement, TGCG keeps their partners informed about the outcomes of all live-in referral enquiries. This ensures TGCG’s trusted partners can offer ongoing support and monitor their clients to ensure they have found the enhanced care they need.
In the case of Mrs H, who is living with dementia, having a trusted partnership was key. With relevant permissions, Rachael worked closely with TGCG Care Manager, Kerry, sharing 12 months of historical person centered and needs led information. This was invaluable to TGCG when building a care plan that reflected not only Mrs H’s care needs, but who she was/is as a person; her preferences and the important people who are in and have shaped her life. This information is fundamental when working with someone with dementia; to enhance reminiscing, avoid anxiety and facilitate a meaningful day. This information also helped find the right professional care team to work with Mrs H and helped the client/carer match that both TGCG and AAHC strive to achieve.
Working in collaboration also meant that advocate family members of Mrs H did not have to repeat historical information. Making the transition to joint working smoother and less anxious for them.
Whilst working jointly at Mrs H’s home it was important that both TGCG and AAHC respective teams worked as one to achieve the best outcomes. Rachael comments,
“Transitioning from hourly to live-in care for Mrs H was managed well, with the introduction of the TGCG live-in carer and continued daily visits from her existing hourly carer, to support the live-in carer and the transition process.”
TGCG Care Manager, Kerry, worked closely with Rachael to ensure that their care teams were well supported and continuity of faces was managed with a small, consistent team. Kerry and Rachael reviewed Mrs H’s care notes and carer feedback weekly, identifying subtle changes so that the respective care plans were aligned and care delivered was mirrored across AAHC hourly support visits and TGCG’s live-in carer. Both parties maintained open, honest communication lines, which ensured Mrs H received the best care and outcomes that upheld her dignity, independence and choice.
Consistency and continuity is important for everyone, especially for those people living with dementia. A trusted partnership approach with TGCG enables AAHC to refer clients with confidence. It enabled Mrs H to live in her own home with familiar surroundings and familiar faces and receive the care she needed; delivered by a small dedicated team, who in turn have a large multidisciplinary team behind them.
