The stages of intermediate care
The National Institute for Health and Care Excellence (NICE) outlines the four stages of your intermediate care journey.
Before your intermediate care begins you will be assessed by a healthcare professional and discuss your wishes, needs and abilities. Your family will also be consulted, if you wish them to be. You will also receive information about advocacy services available.
Intermediate care is implemented quickly, from a few hours for emergency care to a few days for other care settings. You will receive all the information you need regarding the service and the goals you have set to achieve. You will also be given a list of activities which are deemed risky and you can decide what support you require. This will also be the opportunity to ask questions.
During your intermediate care package you will receive support from a range of professionals, to help you achieve your goals. The length of your care package will adjust in line with your care needs and your progress will be documented daily in your intermediate care plan.
At the end of your intermediate care plan, if you still require additional support your plan will be transferred to another service and you will be provided with all the information about your new plan.

We also have our own in-house Occupational Therapist (OT) who works closely with healthcare professionals and our care teams. Our OT provides guidance and advice that enables people to live well in their own home with any equipment they may need.
